The new teams are:
Cara / Green: Kaylee, Austin, Ken
Brett / Red: Jen, Courtney, Justin
Jillian / Black: Rulon, Hannah
Bob / Blue: Irene, Moses, Olivia
But there's another surprise coming - as the Blue Team is one member short, an eliminated player will be given the chance to return! Hannah and Rulon are tasked with choosing who they want on their team. Hannah suggests Arthur but Rulon isn't convinced. He wants a teammate who is determined, dedicated and devoted to changing their life, and thinks Jay is a better option. Jillian says that she can't make the decision for them but agrees Jay would be a strong teammate, although warns that he could also win the entire show.
Jillian treats Hannah and Rulon to a day away from the ranch, inviting them to her zen-like Malibu beach house to relax. Of course, there is a work-out to contend with first, as she makes them jump blocks and exercise on the beach. Jillian then orders Hannah to run three miles up the beach while she talks with Rulon. He opens up about his wife and comfort eating: "I felt like I was filling a void in my life - true love. Even after [getting married]... It became about me even being embarrassed to be intimate with her; I was big, she is tall, beautiful and gorgeous. It's about me seeing I've been the one sabotaging it to this point."
Later that day it's time to find out who will be joining the Black Team - it's Jay! He started the show at 400lbs and has since lost 111lbs. "For me, this is like a miracle, the opportunity to get back," he says, following five weeks at home. "It makes me feel responsibility to do a lot to pay back this opportunity."
The Challenge
Each team has to collectively balance a pulley representing their combined total weight loss above a tank of water. If they drop the pulley, the tank will smash and they will be out of the challenge. The winning team will receive video chats with relatives. Austin points out that he doesn't have a wife or children, so other people are more deserving of the prize, leading the Green Team to unanimously drop their pulley. Eventually the Red Team win. However, they donate their videos to the Blue Team, reasoning that Courtney just saw her mother, Jen is with her father, and Justin is a good guy.

The Weigh-In
Jay has immunity because he was weighed-in on his return to the ranch, so his weight won't count towards the Black Team total.
Black Team:
Jay: -7lbs
Rulon: -5lbs
Hannah: -4lbs
Blue Team:
Moses: -3lbs
Olivia: -5lbs
Irene: -5lbs
Red Team:
Jen: -4lbs
Courtney: -3lbs
Justin: -2lbs
Green Team:
Kaylee: 0
Austin: -3lbs
Ken: -7lbs

A member of the Red Team will still be going home, although Jen has immunity as the team's biggest loser. Justin immediately says that he wants to leave over Courtney. "She needs to finish her journey," he says. "Don't dream of keeping me here." Courtney breaks down, telling Justin he is the leader and she doesn't want to take that away from everyone. He suggests maybe, in that case, it's time for him to step away: "Let's do what we need to do." Everybody with the exception of Rulon follows his wish and sends him home.
Justin tells everyone that he is a better person because of them and the experience. "This show has saved my life and given me the opportunity to be proud of who I am," he says. In a post-elimination video we learn that he has gone from 365lbs to 225lbs, and since returning home he has been working with his community to change lives.
